I just burned up four of the STMicro L165 power op amps driving with a 0.5V amplitude square wave at ~1380 Hz using the Unity Gain circuit suggested in their datasheet.  I was powering with +/- 15VDC and it seemed to work initiially, but saw a current draw of about 800 mA (versus a spec of 40 mA), increasing to 1.6 A after it was toast.  As far as I can tell I have set up their unity gain configuration correctly, but the 1k across the inputs seems kinda odd and the 0.22 uF cap seems a kinda big to be swinging around.  Anyone have any insight on what I've done wrong?  I generate the input square wave with a couple of TL082's set up as a relaxation oscillator passed through a variable gain attenuator then summed with a DC offset.  I've been starting out with a low amplitude square wave with no DC shift while breadboarding the circuit, though.  The square wave generation works fine.  Ultimately I'm trying to drive a couple of Peltier modules in series, hence the need for a power op amp.

It would be difficult to troubleshoot your circuit without seeing a schematic and even pictures of your circuit would be of great help. You mention driving Peltier modules in the future, are you currently driving modules or otherwise, can you describe your load?  If driving Peltier or Thermoelectric Cooler, what part numbers are you using? We appreciate any feedback you can provide regarding your application.  Thank You

Did it get hot first?  If you overheat the chip, it can start to cross-conduct and then run away and fry itself.

Have you considered a PWM solution for driving the modules?  It would be far more efficient.

I have to agree with DVasend that it's difficult to troubleshoot without seeing a schematic.  What was the load that was on the output of the OpAmp?  Did you try it with the Peltier modules you're going to use?

As long as your output of the TL082's is good, the input to the L165 should be fine.  I do agree with you about the 1K resistor across the inputs looks odd.  Did you try it with that 1K between ground and the non-inverting input?
